SERIES OF SAFETY PLANNING MEETINGS

  • Case Manager to complete referrals to appropriate Care Team Members, clearly indicating what support is needed and the timelines in order to achieve specific Safety Goals
  • Case Manager and Care Team Members meet to complete Safety Panning Worksheets
  • Case Manager to schedule regular Review Meetings to develop/amend plan using the rules/strategies the family/youth has developed through their work with the Case Manager and members if their Care Team
  • La Paraantii included in all planning meetings
  • Captain of the Plan is identified by family/youth and is presented with a Métis Sash
  • Safety Scale(s) reviewed and scaled with participants. Each participant's number and rationale are documented
  • Honour successes and identify challenges. Adopt the rules/strategies to address challenges to increase safety
  • LMO Life Promotion Plan created as immediate safety plan for youth (if applicable)
  • Kwaayesh Ni Kaanawaymikawin (Connection and Belonging) - final column used to move from Interim to long-term plan created for children/youth in care and youth in agreements
