MONITORING THE PLAN

• All members of the Care Team, including La Paraantii, are always included

• Captain of the Plan taking a bigger lead, reporting on the progress of the plan

• Review of Safety Scale(s) with Participants. Participant's number and rationale documented

• Honour successes and identify challenges.

• Adapt the rules/strategies when needed to increase safety

• Unpacking the safety journal or other monitoring tools used

• Family and La Paraantii decide which rules/strategies from the adult plan go into the child's plan
